- Rehab Therapist specializing in Physical Therapy Asst credentialed from the Oklahoma Board of Medical Licensure and Supervision.
- Physical Therapist Assistant temporary letter will be accepted until license is issued.
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience.
