  • Demonstrates independent nursing actions based on assessment and problem identification
  • Calls in prescription medications to pharmacies and observes and documents patient’s response to pertinent medications
  • Documents accurate and ongoing assessment of patient’s needs, problems, capabilities, limitations, progress toward goals, and response to care
  • Triages telephone calls and initiates appropriate nursing intervention

Qualifications

Required

  • Current Connecticut RN License
  • Previous experience in Medical/Surgical Office setting preferred
  • Experience with medical software

Job Description

The structural heart program provides expert multidisciplinary care for abnormalities of the heart and great arteries, including the heart muscle, heart valves, and supporting structures. Procedures include TAVR, TMVR, MitraClip, and LAA implants. The Structural Heart Nurse is a valued member of this provider led team serving as a liaison to our patients.

Under provider direction, the RN provides skilled nursing care to patients in a variety of clinical settings. Scope of responsibility is characterized by use of nursing process to assess, plan, intervene and evaluate human responses to actual or potential health problems utilizing appropriate practices, standards, protocols and guidelines. This position reports to a Practice Manager.

  • Helps to facilitate appointments for consults, clinic, and support services (both inpatient and outpatient)
  • Communicates and assists in coordinating patient care with leads of cardiology team in order to ensure proper transition of care.
  • Ensures that data is completed appropriately for registries.
  • Provides office-based nursing care in collaboration with provider
  • Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention
  • Communicates with provider regarding patient needs, nursing assessments, and recommendations
  • Takes direction and initiates actions (cross/additional training) that will allow seamless patient care
  • Collects and documents current health status and medical history and assists in scheduling patient office visits
  • Reviews patient records, including preoperative records, to ensure completeness and reviews outstanding issues with provider
  • Demonstrates independent nursing actions based on assessment and problem identification
  • Under direction of provider, calls in prescription medications to pharmacies and observes and documents patient’s response to pertinent medications
  • Documents accurate and ongoing assessment of patient’s needs, problems, capabilities, limitations, progress toward goals, and response to care
  • Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner
  • Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy

Qualifications

Position is onsite at SVMC with Structural Team

  • Current Connecticut RN License required.
  • Previous experience in Medical/Surgical Office setting preferred.
  • Experience with medical software
