Job Summary and Qualifications
The Transplant Clinical Nurse Coordinator (CNC) is a hybrid Charge RN/Leadership role that functions to deliver high quality, patient-centered care while overseeing the coordination of department processes. In collaboration with other members of the team, the CNC directs, monitors, and evaluates nursing care in accordance with established policies/procedures, serves as a clinical resource person for patients and bedside staff, and models a commitment to the organization’s vision/mission/values to support and maintain consistently excellent patient experience and quality clinical outcomes for a specialized patient population.
