HCR Home Care is actively hiring Certified Home Health Aides for patients in Warren and Washington Counties.   • Immediately report changes in client's condition to supervising professional.
  • Complete and submit necessary documentation including charting tasks and observations on android device.
  • Attend ongoing in-service programs in a timely manner to maintain certification.
  • All other duties as assigned.

Qualifications

Required

  • Valid NYS Home Health Aide Certificate (must have worked for a Licensed Home Care Agency within last 24 months)
  • Ensure the safety of the patient, patient's family and/or the employee
  • Maintain HIPPA
  • Valid NYS Driver's license and own reliable transportation required
  • Communicate effectively with patient, family and interdisciplinary team
  • Ability to visually recognize objects within 20 feet
  • Ability to speak and hear
  • Frequent walking and standing (80%-100% of the time)
  • Frequent lifting
  • Must be able to lift / move 40 lbs. unassisted
  • May require pushing / pulling / transferring 10 lbs. of weight
  • Sufficient hand strength and dexterity to carry out care plan duties
  • Ability to bend, stoop, reach, pull and push unsupported, routinely
  • Ability to climb two flights of stairs

Warren or Washington County Certified Home Health Aides - HCR patients need you!

We are actively hiring HHAs who can give us regular weekly availability of 24-40 hours per week!

Scheduled hours to be worked between the hours of 8am and 5pm Mon-Friday with occasional weekend hours.

Starting rate is $18.55/hr. plus additional incentive pay available

Position Summary:

Provide personal care and supportive services to individuals in the community in the privacy of their home. Must possess unexpired NYS certification as a Home Health Aide, in good standing, according to NYS Dept of Health regulations. Ability to work effectively within scope of job. . Must be respectful, compassionate and empathetic.
