The Acute Care Hospital At Home (ACHAH) NP/PA will provide care and oversight for patients enrolled in ACHAH, including in-home and telemedicine visits, as well as inpatient rounding. Qualifications
Required
- NYS licensed/registered NP or PA.
- Master's Degree as a Nurse Practitioner or Physician Assistant.
- Current National Certification consistent to the clinical practice.
- NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).
- CPR-BLS Certification required at the time of appointment.
Preferred
- 1-2 years NP/PA experience in Medicine, Acute Care preferred.
