The Acute Care Hospital At Home (ACHAH) NP/PA will provide care and oversight for patients enrolled in ACHAH, including in-home and telemedicine visits, as well as inpatient rounding. Responsibilities

  • Maintain outstanding relationships with families, referring physicians, and all members of the health care team.

Qualifications

Required

  • NYS licensed/registered NP or PA.
  • Master's Degree as a Nurse Practitioner or Physician Assistant.
  • Current National Certification consistent to the clinical practice.
  • NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).
  • CPR-BLS Certification required at the time of appointment.

Preferred

  • 1-2 years NP/PA experience in Medicine, Acute Care preferred.

Responsibilities include significant interdisciplinary collaboration, providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care assessing & evaluating patients, ordering appropriate laboratory testing & diagnostic studies, arranging for referrals, testing, consultation & therapeutic services, and maintaining outstanding relationships with families, referring physicians & all members of the health care team.

Minimum Qualifications:

NYS licensed/registered NP or PA required. Master's Degree as a Nurse Practitioner or Physician Assistant & current National Certification consistent to the clinical practice also required. NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire).

CPR-BLS Certification required at the time of appointment.

Preferred Qualifications:

1-2 years NP/PA experience in Medicine, Acute Care preferred.

Work Days:

Monday - Friday, Days, with weekend and holiday rotation.
