The Occupational Therapist provides occupational therapy services to individuals as outlined in their Individual Education Plan or Individual Family Service Plan, in accordance with New York State practice act guidelines, the American Occupational Therapy Association, and all company, state, and federal standards and regulations.   • Works as part of an inter-disciplinary team, utilizing communication with all team members to enhance the treatment program of the individual
  • Maintains an accurate schedule
  • Submits all required documentation of visits according to established procedures
  • Performs evaluations as assigned and when necessary
  • Communicates with families on a regular basis
  • Keeps abreast of legal responsibilities, professional ethics, standards of practice and agency rules, policies and regulations
  • Provides for own professional growth by attending continuing education and in-services; tracking this to assure compliance with state licensing requirements
  • Discontinues therapy when child no longer meets eligibility requirements
  • Provides own transportation to service locations
  • Maintains client confidentiality with HIPPA/FERPA and agency guidelines
  • Maintains professional appearance, follows agency dress code
  • Attends annual fee for service meeting and any other meetings required by your supervisor
  • Attends CPSE and IFSP meetings for individuals on caseload as required
  • Adheres to and signs Annual Employee Agreement
  • Documents time off and time worked accurately according to agency policies

Qualifications

Required

  • Graduate of an accredited Occupational Therapy program with a Bachelor's Degree
  • 1-2 years of pediatric experience
  • New York State Occupational Therapy License and Registration
  • Valid NYS Driver's License
  • Minimum of 36 hours of Continuing education per licensing period (3 years) as per NYS Office of Profession guidelines
  • All required agency trainings
  • All trainings required by programs you service individuals in, including but not limited to, OCFS and Early Intervention

**Job Summary:**

Provide occupational therapy services to individuals as outlined in their Individual Education Plan or Individual Family Service Plan. Services are to be provided in accordance with New York State practice act guidelines for Occupational Therapy Services, the American Occupational Therapy Association as well as in accordance with all company, state and federal standards and regulations; as well as within all regulations and requirements set forth by the organizations with which the company collaborates.

**Essential Job Duties:**

**Physical Requirements:**

All services rendered will be paid at the Fee For Service (FFS) rate, per the service provided.
