- Serve as mentor and collaborating physician to nurse practitioners for those states where it is required
Qualifications
Required
- Medical Degree
- 2 years Post residency and primary care experience
- Knowledge of disease prevention and disease management
- Knowledge of patient rights and laws relative to those rights, such as HIPAA
- Excellent communication skills both written and verbal
- Ability to build comprehensive treatment plans and effectively convey information to others
- Current knowledge of clinical practices and related medical policies
- MD or DO - Physician - State Licensure Upon Hire
Preferred
- Virtual care delivery experience
- Knowledge of computer-based systems, as well as medical records and point-of-care technology

**PURPOSE:**
We are currently hiring physicians to join our CloseKnit team to deliver care. CloseKnit is a virtual first advanced primary care practice looking for an experience person interested in innovating in the practice of primary care to deliver on the quadruple aim in a value-based arrangement for adult and pediatric patients. While providing mainly virtual primary care services, the physician will be working with the CloseKnit team to optimize the care delivery strategy and best practices in delivering a hybrid model of care to our patients.
**ESSENTIAL FUNCTIONS:**
**Primary Care Services**
+ Maintain a panel of patients who receive high quality, attentive, longitudinal and virtual primary care
+ Provide comprehensive primary care including diagnosis, treatment and, documentation for acute, preventive and chronic patient needs.
+ Work with both patients and their families.
