  • Maintains the Community Health Outreach grid and ensures that all vital information has been captured and denoted, including # of attendees, # of RNs and MDs participating and supportive strategy
  • Provide a monthly department report of activities to the director
  • Quantifies and/or reviews the community benefit for each event that RWJUH Somerset participates
  • Enters community benefits for each event in the CBISA system
  • Obtain and update the fringe benefit annually
  • Obtain and update average salary and conference room expenses
  • Work with other departments and/or personnel to obtain expenses from outreach events
  • Classify programs appropriately in the CBISA system
  • Maintain supporting documentation of community benefits
  • Educate department and staff on current community benefit regulations
  • Provide community benefit in-services to departments to increase knowledge
  • Distribute quarterly reminders to report community benefits
  • Facilitate department reporting processes

Qualifications

Required

  • Undergraduate degree in related field

Job Summary:

Essential Functions:

+ Prepares presentations, handouts and evaluations along with educational materials for events in which presenting

+ Assists with grants as needed

+ Develops outreach campaigns for strategic hospital initiatives including but not limited to mammograms, lung screenings, diabetes, colonoscopy, etc

+ Run EPIC reports to identify individuals who are eligible or need screenings and/or preventive services

+ Sends routine screening reminders (via mailings, MyChart, etc.) to schedule appointments

+ Tracks effectiveness of outreach and drafts appropriate reports

+ Represents and promotes Community Health at internal and external events (i.e. health screenings, exhibits, community functions)

+ Supports community events during non-regular working hours (i.e. after 5 p.m. and on weekends)

+ Collaborates with the Outreach Specialist/Coordinator to ensure professional table exhibits at events

+ Acts as the hospital liaisons at health fairs and other events
