  • Identify resident problems and concerns and report them immediately to the Charge Person.
  • Conduct resident rounds as assigned.
  • Assure residents have call lights at hand and answers call lights promptly.
  • Implement corrective action as necessary under the direction of RN Case Manager.
  • Report and record incidents and/or unusual problems according to established facility procedures.
  • Report to Charge person, the care provided to the resident and the resident's response or lack of response to care provided.
  • Report changes in residents' conditions to RN Case Manager.
  • Complete documentation utilizing computerized medical record system accurately, timely and following facility policies and procedures.
  • Refers resident and family complaints and problems to Charge person.
  • Assure residents are monitored with meals, nourishment and transportation to meal service area.
  • Provide housekeeping services to residents.
  • Provide other supportive tasks for residents as needed.
  • Follow facility procedures for admission, discharge and transfer of residents.

Qualifications

Required

  • High school diploma or equivalent.
  • Must have HHA or PCA Certification.

Full Job Description

HHA/PCA - Part Time - 3rd Shift

Regular Part-Time

Jamestown, NY, US

Lutheran's Hultquist Place, an assisted living facility, is currently seeking a compassionate and caring Certified Home Health Aide (HHA) or Personal Care Assistant (PCA) for a Part Time, 3rd shift position, working 10pm-6am every other weekend, at minimum.

Salary Range: $16.70 Minimum to $22.92 Maximum.

Purpose: Our HHAs and PCAs provide direct care to residents as indicated by the individual care plan, under the supervision of the RN Case Manager in accordance with facility policies and procedures and report resident needs and concerns to the RN Case Manager. This person is also responsible for medication administration as well.

Qualifications:

High school diploma or equivalent required. Must have HHA or PCA Certification, which means the successful completion of New York State training for home health aide services or equivalent exam approved by New York State is required. Knowledge of data entry in computer for medical records.
