Responsibilities

  • Be directly responsible to Program Manager for admission and discharge issues while conforming with the agency's program definition and policies relevant to program/client needs.
  • Screening, assessing and developing service plans.
  • Developing/Monitoring the implementation of strength-based case plans and progress towards goals.
  • Assisting clients in accessing community resources for needed assistance utilizing Community Cares closed loop referral system for the State of Arizona.
  • Facilitating and coordinating discharge planning transition and follow up according to plan.
  • Recognizing common symptoms of mental disorders and/or addictions.
  • Recognizing the indications, common side effects, reactions and interactions of psychotropic medications prescribed for self-administration by agency clients
  • Coordinating services with clients/families and other service providers.
  • Providing educational information as needed to assist clients/families in reaching goals.
  • Attending and participating in court hearings as required to meet program needs and within scope of practice.
  • Performing intake screenings, lethality assessments and basic needs assessments for potential clients (as relevant to program/client needs).

Qualifications

Required

  • High School Diploma or GED
  • Bilingual skills required (Spanish verbal/written)
  • 1-2 years of direct Case Manager Experience

Preferred

  • Associate or Bachelor's Degree in a related field
  • Experience working with individuals experiencing homelessness or those who are economically disadvantaged
  • Knowledge of community resources
  • HMIS experience

A New Leaf's Homeward Bound Shelter Program is seeking a dedicated Bilingual Case Manager to join our team. This role involves providing comprehensive case management services to individuals and families experiencing homelessness. The Case Manager will conduct assessments, develop and implement individualized service plans, and connect clients with community resources to support their journey towards self-sufficiency and stable housing. Hourly Pay Range: $18 - $21 (depending on years of experience)

Location: Phoenix, AZ (on-site role at the family shelter)

Schedule: Monday - Friday; either 8am - 4pm or 9am - 5pm (flexibility needed for client needs)

QUALIFICATIONS

Education:
  • High School Diploma or GED required
  • Associate or Bachelor's Degree in a related field, preferred
  • Bilingual skills required (Spanish verbal/written)

  • Class One Fingerprint Clearance Card
  • Valid Driver's License with reliable transportation
  • Current Auto Liability Insurance
  • CLEAN 39 Month Motor Vehicle Record
  • Must Be At Least 21 Years of Age
