The Case Manager at Hope Network coordinates all services necessary to maximize consumer recovery and independence for individuals with mental illness. Responsibilities

  • Ensure consumer involvement in the Person Centered Planning Process (PCP).
  • Facilitate the implementation of Individual Plan of Service.
  • Perform comprehensive, ongoing psychosocial assessments.
  • Participate in legal proceedings including petitioning and testifying in clinical emergencies.
  • Provide community based services.
  • Facilitate the coordination of consumer intakes, discharges, and transfers.
  • Communicate with all service providers in Treatment Plan.
  • Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
  • Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
  • Provide referral to appropriate community resources.

Qualifications

Required

  • Bachelor’s degree in a human services field from an accredited institution.
  • Prior experience working with Developmentally Disabled population.
  • Valid Michigan Driver’s License.

Preferred

  • LLBSW, LBSW, LLMSW, LMSW.
  • Prior case management experience in a mental health or rehabilitation setting.

Full Job Description

This position does require working in the community and performing home visits.
