  • Documenting patient care interactions within an EMR template for program oversight and growth.
  • Developing psychosocial programming specific to the AYA population including support groups and other therapeutic interventions.
  • Using specialized clinical knowledge skills in psychotherapy, clinical counseling, diagnostic interviews, assessments and treatment plans to patients.
  • Other duties as assigned.

Qualifications

Required

  • At the Licensed Social Worker (LSW)/Clinical Social Work Intern (CSW-I) level, assessment, and treatment skills as obtained by the Master's in Social Work.
  • At the Licensed Independent Social Worker (LISW)/Licensed Clinical Social Worker (LCSW) level, assessment, diagnostic and treatment skills as obtained by the LISW/LCSW.
  • State Licensed Social Worker (LSW), Clinical Social Work Intern (CSW-I), Licensed Independent Social Worker (LISW), or Licensed Clinical Social Worker (LCSW) license as state requires.

Preferred

  • Oncology experience
  • Ability to utilize other electronic medical record templates and computer software such as SmartForms, electronic dashboards, and spreadsheets

We are looking for an Adolescent and Young Adult (AYA) Social Worker Navigator to join our team. This role is brand new, and we are looking for a caregiver that would be up for the challenging yet rewarding work. You will be focusing on oncology patients ages 15-39 years that have a wide variety of psychosocial needs, supporting Cleveland Clinic's mission of caring for life.

**Complexity of Work:**

+ Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

+ Must be able to work in a stressful environment.

**Work Experience:**

+ At the LSW/CSW-I Level: Master's degree in Social Work. Ohio/Nevada License. LSW/CSW-I. May have experience in hospital care or closely related field as employee, graduate or undergraduate intern.

+ At the LISW/LCSW Level: Master's degree in Social Work. Ohio License at the Independent level (LISW), Nevada license at the Licensed Clinical Social Work level (LCSW). Requires two years of post graduate employment with clinical supervision

+ Oncology experience is preferred.

+ The ability to utilize other electronic medical record templates and computer software such as SmartForms, electronic dashboards, and spreadsheets preferred.

**Physical Requirements:**

+ Ability to sit/stand for extended periods of time.

+ Must be able to climb stairs, pull, push and facilitate movement of the patient.

+ Must have normal or correctable vision.
