The Quality Management Coordinator at Mercy Hospital is a Registered Nurse responsible for collaborating with the nursing staff on strategies for Sepsis and Core Measures Compliance, compiling and analyzing data to promote effective quality care, ensuring regulatory survey readiness, and partnering with an interdisciplinary team to optimize patient outcomes. This role involves advocating for patients and their families, participating in safety initiatives, and contributing to overall Quality/Performance Improvement activities.

Responsibilities

  • Collaborate with the Nursing Staff on strategies for Sepsis and Core Measures Compliance and educational efforts to support success.
  • Be responsible for gathering, compiling, analyzing data and producing information to promote effective Quality Care.
  • Prepare to ensure regulatory survey readiness, with knowledge of Joint Commission, CMS and NYSDOH standards and regulations.
  • Partner with an interdisciplinary team including, but not limited to, attending physicians, Registered Nurses (RN), and ancillary staff to optimize patient outcomes.
  • Advocate for and build meaningful relationships with patients and their loved ones through safety intervention, emotional support, and education.
  • Participate in safety initiatives at the department level to improve patient care outcomes.
  • Provide real time accountability, coaching, education to promote ownership of safe practice.
  • Complete chart reviews for mortalities, readmissions, quality issues, peer review, etc.
  • Report to Medical Staff and Administration on a regular basis, findings, trends and opportunities for improvement for Sepsis and core measures.
  • Participate in overall Quality/Performance Improvement activities as directed.

Qualifications

Required

  • Graduate of an accredited School of Nursing
  • Bachelor of Science in Nursing (BSN) degree
  • New York Registered Nurse (RN) License
  • Skilled in the use of electronic medical record (EMR), MS Office, MS Excel, & MS PowerPoint

Preferred

  • 4+ Years of RN experience working in an acute care hospital
  • Experience in Quality/Performance Improvement

You are at the heart of health Job Details The Stroke Program Coordinator is responsible for coordinating the Stroke and Core Measures programs at Mercy Hospital Our Stroke Coordinator will: Be responsible for the development and maintenance of the Stroke and Core Measures program at Mercy Hospital Have knowledge of current disease specific to Clinical Practice Guideline's and evidence-based stroke care Perform all aspects of data collection, abstraction, analysis, and reporting as related to the Stroke and Core Measures program to promote Quality Care Take a lead in providing staff with Stroke and Neurology Education, providing real time accountability, coaching, and education to promote ownership of safe practice. Participate in community outreach to educate the community on the importance of stroke prevention and post stroke care. Participate and assist with Stroke support group Communicate, discuss and review cases with Stroke Medical Director Lead monthly stroke committee meetings and provide minutes Prepare to ensure regulatory survey readiness, with knowledge of Joint Commission, CMS and NYSDOH standards and regulations Partner with an interdisciplinary team including, but not limited to, attending physicians, Registered Nurses (RN), and ancillary staff to optimize patient outcomes. Ensure all staff working on the team, have their required credits to care for stroke population Advocate for and build meaningful relationships with patients and their loved ones through safety intervention, emotional support, and education, including and connecting with them post discharge Actively participate in Quality Audits, Patient Experience/Leadership Rounds, and must be willing to cross train on all Quality initiatives Salary Range USD $55.01 - USD $70.28 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
