Green Meadows Home Health is seeking a Licensed Vocational Nurse (LVN) to join our Home Health Case Manager team. This full-time position involves managing patient care, ensuring eligibility for services, and coordinating with healthcare professionals to provide the best possible care. Responsibilities

  • Intake and referral processes
  • Check eligibility for all available services
  • Enter and fill up all required fields on patient profile
  • Initially staff the case accordingly
  • Notify patients/families receiving home health services
  • Validate patient address, contact numbers, and provide name of discipline performing SOC
  • Ensure supplies are available in patient's home prior to SOC
  • Inform SOC discipline to show their ID and insurance cards to patients
  • Remind SOC discipline to ask patients to get all medication ready
  • Respond/answer all emails addressed to you
  • Note after having frequency: plot/task the correct discipline till the end of the episode
  • Ensure all authorizations are provided for staffing/tasking
  • Follow up on all lab requests with PCP/pharmacy name faxes to receive results
  • Follow up on all DME orders
  • Complete all reports with comm notes/telephone orders completed in patient charts

ENTERED ANY "FLAGS" NEEDED TO CALL DISCIPLINE ATTENTION
INITIALLY STAFF THE CASE ACCORDINGLY
EMAIL CARE COORDINATORS/MANAGERS RE:
1. NAME OF SOC STAFF
2. DATE OF SOC
B. CASE MANAGERS / CASE COORDINATORS
READ /RESPOND TO INTAKE /REFERRAL EMAIL
CHECK PATIENT PROFILE COMPLETED AND ALL FIELDS ADDRESSED
VALIDATE THE SOC DISCIPLINE ACCEPTED TASKED PATIENT ASSIGNED TO THEM
VALIDATE SOC DATE IS AGREED UPON BOTH FIELD STAFF AND THE AGENCY
CALL PATIENT/FAMILY NOTIFY THEM, RECEIVING HOME HEALTH SERVICES, ORDERED BY....?
VALIDATE PATIENT: ADDRESS, CONTACT NUMBERS, PROVIDE NAME OF DISCIPLINE
PERFORMING SOC AND DATE THIS ENCOUNTER WILL HAPPEN
IF SUPPLIES IS NEEDED, ENSURE SUPPLIES ARE IN PATIENT'S HOME PRIOR TO DISCIPLINE PERFORMING SOC ARRIVE IN PATIENT RESIDENCE
INFORM DISCIPLINE DOING THE SOC, ALL SUPPLIES ARE AVAILABLE IN-PATIENT HOME TELL PT/FAMILY TO SHOW THE DISCIPLINE WITH THEIR ID AND INSURANCE CARDS
REMIND THE SOC DISCIPLINE TO ASK FOR PT/FAMILY TO GET ALL MEDICATION READY (EVEN WITH REFRIGERATOR: INSULIN), ID AND INSURANCE CARD OF PATIENTS
REMIND SOC DISCIPLINE TO EMAIL SHORT REPORT ABOUT PATIENT FINDINGS, FREQUENCY OF VISITS, AND ENTERED ALL THEIR MEDICATIONS ON CHART
RESPOND/ANSWER ALL EMAILS ADDRESSED TO YOU ALL
C. CASE MANAGERS/CARE COORDINATORS NOTE:
AFTER HAVING FREQUENCY: PLOT/TASK THE CORRECT DISCIPLINE TILL THE END OF THE WHOLE EPISODE
ALL AUTHORIZATIONS MUST BE PROVIDED FOR THE STAFFING/TASKING GETS COMPLETED
FOR THE WHOLE EPISODE. ENCOURAGE/REQUEST REPORT FROM YOUR DISCIPLINE IF THERE WAS MVR(AT ANY TIME) THERE SHALL BE NO MVR (7 DAYS WEEKLY)
EVERY WENESDAY, CHECK ALL PATIENTS THAT REQUIRED REPLESHNESHMENTS FOR THEIR SUPPLIES/ MEDS. TRY TO DELIVER THIS BY FRIDAY
1. NO LAB DRAWS ON WEEKENDS AND HOLIDAY
2. ALL LAB REQUEST MUST HAVE PCP / PHARMACY NAME FAXES TO RECEIVED RESULTS
3. DO FOLLOW UP ALL LABS THE FOLLOWING DAY AFTER DRAW
4. FOLLOW UP ON ALL DME ORDERED
ALL REPORTS MUST HAVE COMM NOTES/ TELEPHONE ORDERS COMPLETED IN PATIENT CHARTS
5. FOLLOW UP ON ALL THE LABS DRAWN THE DAY AFTER...VERIFY LABS DRAWN, REQUESTING
DEPT RECEIVED ALL RESULTS. DOCUMENT ON COMM NOTES ALL OF THIS ACTIVITIES
