The Physical Therapist at Amergis is responsible for performing patient evaluations, providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician's orders.   • Assesses muscle strength, mobility, gait, and Range of Motion (ROM).
  • Removes staples or sutures and manages incision line on orthopedic surgery incisions.
  • Provides physical therapy treatment and establishes physical therapy plan of care.
  • Instructs patient, families, and/or caregivers in the use of and care of therapeutic appliances.
  • Determines priority needs for physical therapy.
  • Reports patients' reaction(s) to treatment or changes in condition to physician and/or designee.
  • Initiates physical therapy program and instructs other personnel and/or family/caregiver regarding certain phases of physical therapy.
  • Trains patient in use of prosthetic device(s) and other assistive devices.
  • Uses appropriate equipment to meet patient goals and maintains equipment according to manufactures guidelines.
  • Prepares and submits clinical and progress summaries.
  • Participates in discharge planning and peer consultation process.
  • Supervises Physical Therapy Assistants according to company policies and state regulations.
  • Provides/submits accurate and timely documentation in accordance with the plan of care.

Qualifications

Required

  • Current licensure as a Physical Therapist in the state of practice
  • Graduate of an accredited school of Physical Therapy
  • Complies with all relevant professional standards of practice
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB Questionnaire, PPD or chest x-ray if applicable
  • Current Health certificate (per contract or state regulation)
  • Must meet all federal, state and local requirements
  • Successful completion of new hire training as applicable to job site
  • Understand patient confidentiality and HIPAA requirements
  • Ability to effectively communicate via strong communication skills; proficiency in the English language is required
  • Computer proficiency required
  • Must be at least 18 years of age

Preferred

  • One (1) year of prior professional Physical Therapy experience

Full Job Description

The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physician's orders when applicable.

Essential Duties and Responsibilities:

+ Performs initial comprehensive assessment and reassessments and regularly reevaluates patient for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriate

+ Participates in developing the plan of care in collaboration with the physician and other members of the health care team

+ Assesses and evaluates and/or assists the physician in assessing and evaluating therapeutic, rehabilitative, function status, and home environment (as applicable)

+ Assesses muscle strength, mobility, gait, and Range of Motion (ROM)

+ On incisions resulting from orthopedic surgery, removes staples or sutures and manages incision line

+ Provides physical therapy treatment and establishes physical therapy plan of care

+ Instructs patient, families, and/or caregivers in the use of and care of therapeutic appliances

+ Determines priority needs for physical therapy

+ Reports patients' reaction(s) to treatment or changes in condition to physician and/or designee

+ Initiates physical therapy program and instructs, consults, and/or advises other personnel and/or family/caregiver regarding certain phases of physical therapy with which they may work with the patient, and explains the goals of the physical therapy program by participating in case conferences

+ Trains patient in use of prosthetic device(s) and use of other assistive devices (e.g., walker); ensures equipment used in the course of patient treatment is in good working order

+ Uses appropriate equipment to meet patient goals, cleans and maintains equipment according to manufactures guidelines

+ Prepares and submits clinical and progress summaries based on the attainment and progress of goals, and responses to treatment or changes in condition

+ Participates in discharge planning

+ Participates in peer consultation process

+ Supervises Physical Therapy Assistants according to the company's policies/procedures and state regulations

+ Provides/submits accurate and timely documentation in accordance with the plan of care and company policy and procedures

+ Complies with applicable local, state, and federal laws and regulations regarding professional standards and practice standards

+ Performs other duties as assigned/necessary

Minimum Requirements:

+ Current licensure as a Physical Therapist in the state of practice

+ Graduate of an accredited school of Physical Therapy

+ One (1) year of prior professional Physical Therapy experience preferred

+ Complies with all relevant professional standards of practice

+ Participation and completion of Amergis' Competency program when applicable

+ Current CPR if applicable
