Required

  • Master’s degree in social work from an accredited school of social work
  • 3-5 years in health-related field, hospital experience preferred
  • In Patient Experience Required
  • Behavioral Health background preferred

Job Title: MSW Social Worker

Job Location: Summit, NJ

Job Duration: 1 Month+ (Possible Extension)

Shift: 8 am – 4 pm or 8:30 am - 4:30 pm

Job Summary:

· Collaborate with a multidisciplinary team to deliver high-quality, patient-centered care.

· Address social determinants of health to support safe care transitions and improve outcomes across the facility.

Job Responsibilities:

· Patient Assessment, Referrals, and Intervention (60%): Conduct psychosocial assessments, address barriers to care, and support health and social needs.

· Documentation (20%): Maintain accurate and timely records of assessments, interventions, and referrals.

· Meetings and Program Development (10%): Participate in meetings and contribute to program development.

· Collaboration with Agencies (10%): Coordinate with internal and external agencies for comprehensive patient support.

Job Functions:
