- Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities
Qualifications
Required
- CASAC-T, CASAC, LMSW or LMHC certification
- Master's Degree in related field

- Experience in substance abuse counseling

Arms Acres is looking for a Clinical Case Manager to join our outpatient team in Carmel, NY. In this role, you will provide comprehensive clinical case management services that include: assessments, individual, group and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems, including social service programs and managed care.
+ Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures
+ Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames
+ Documents patient progress and own interventions in case record and works within time frames of regulatory requirements
+ Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director

Requirements
REQUIRED QUALIFICATIONS:
+ CASAC-T, CASAC, LMSW or LMHC
