The Licensed Practical Nurse (LPN) at CARLE's First Care Clinic is responsible for delivering patient care that promotes safety and well-being.   • Advise and answer questions, give results and instructions to patients via telephone calls.
  • Assist provider and other clinical staff with preparing patients for examination, treatment, and minor surgical procedures.
  • Draw and collect blood and other samples from patients for laboratory analysis.
  • Administer vaccines and ordered medications by oral, IM, SQ, or ID routes.
  • Place urinary catheters per protocol.
  • Schedule referrals for patients with specialists or outpatient as requested by the provider.
  • Track system for follow-up appointments related to office visits, laboratory tests, and procedures.
  • Provide appropriate and timely documentation in the patient's electronic health record.
  • Become certified in occupational medicine testing within 6 months of hire date.
  • Maintain compliance with CLIA, OSHA, Safety and Risk Management guidelines.
  • Provide education to patients and families using the Teach-back method.

Qualifications

Required

  • Licensed Practical Nurse (LPN) certification.
  • Ability to perform rooming patients, vital signs, and information gathering for appointments.
  • Competence in clinical skills necessary for assigned job duties.
  • Ability to draw and collect blood and other samples from patients.
  • Ability to administer vaccines and ordered medications.
  • Ability to place urinary catheters.
  • Knowledge of CLIA, OSHA, Safety and Risk Management guidelines.
  • Ability to provide education to patients and families using the Teach-back method.

Preferred

  • Occupational medicine certifications as advised by supervisor within 6 months of hire date.

Full Job Description

Under the direction of a health care provider, Clinical Nurse LPN, is responsible for the delivery of patient care that promotes the safety and well-being of our patients. Including, but not limited to, rooming patients including vital signs, information gathering of information for the appointment and gap risks, vaccination statuses, reason for visit, etc. Perform testing and procedures including nasal swabs, venipuncture, injections, ear lavages, triage per illness or injury severity, TB placements. Occupational medicine testing including drug screens, breath alcohol, audiograms, PFT, vision, and other as directed by provider. Regular maintenance of in-basket and nurse folders per protocols. Telephone calls with patients to advise and answer questions, give results and instructions per provider. Other tasks within scope per clinic needs or advised by supervisor.
  • Assist provider and other clinical staff with obtaining basic vital information preparing patient for examination, treatment and minor surgical procedures.
  • Demonstrates competence in the clinical skills necessary to carry out assigned job duties.
  • May be required to draw and collect blood and other samples from patients and prepare specimens for laboratory analysis.
  • Administer vaccines and ordered medications by oral, IM,SQ or ID routes.
  • Place urinary catheters per protocol.
  • Schedule referrals for patients with specialist or outpatient as requested by the provider. Complete appropriate forms or requisitions needed for referrals or lab testing, including appropriate CPT/ICD-9 coding.
  • Responsible for tracking system for follow-up appointments related to office visits, laboratory tests and procedures to regularly follow patients monitoring disease status.
  • Provide appropriate and timely documentation in the patient's electronic health record using standardized workflows and processes.
  • Become certified in occupational medicine testing as directed by provider or manager with in 6 months of hire date.
  • Maintain compliance with CLIA, OSHA, Safety and Risk Management guidelines.
  • Maintain compliance and behave in a manner consistent with all policies and procedures, including but not limited to Compliance, HIPAA and personnel workplace rules.
  • Demonstrate initiative to improve quality and customer service by striving to exceed customer expectations.
  • Provide education to patients and families using the Teach-back method per protocols and guidelines.
  • Occupational medicine certifications as advised by supervisor within 6 months of hire date.
