Program Description: In Homes Now
In Homes Now is a cluster of scatter site permanent housing programs with 257 apartment units in The Bronx, Manhattan and Brooklyn. The program serves men, women, and families. The individuals and families have histories of homelessness, mental health and substance use. The multidisciplinary team includes case managers, peer counselors, psychiatric care providers, and occupational therapy interns who work with clients to ensure housing stability and improve wellbeing.
Essential Duties and Responsibilities:
+ Escort clients to medical appointments, benefits appointments, and other appointments as needed
+ Carry a small caseload and conduct home visits with clients
+ Assist clients with everyday activities such as grocery shopping and money management
+ Facilitate groups for clients at the office
