Home Health Nurse Responsibilities

• Deliver hands on nursing care in the home, including comprehensive patient assessments, wound care, medication administration, and other skilled services
• Provide patient education to patients and families for a wide variety of diagnoses, ensuring the patient can effectively manage their conditions at home
• Facilitate communication to all members of the patient’s care team, incorporating family members, therapists, social workers, home health aides, and physicians
• Complete accurate and timely documentation of services provided

Minimum Requirements

• Registered Nurse with active, unrestricted license in the State of Michigan – BSN not required
• At least one (1) year of clinical experience as a Registered Nurse
• Current CPR Certification
• Reliable transportation with a valid state driver's license and current car insurance

Preferred Skills

• One (1) year of Home Health experience
• Community health and a wide range of Medical-Surgical experience
