Responsibilities

  • Assess, plan, implement, and evaluate patients' needs and care.
  • Perform duties and responsibilities in a manner consistent with the mission, values, and Mercy Service Standards.
  • Comply with accepted professional standards and principles.
  • Understand Hospice philosophy, issues related to death/dying and grief.
  • Comply with Unity Health Hospice Health Status requirements.
  • Provide good written documentation in a timely manner.
  • Be flexible and cooperative in fulfilling role obligations.
  • Work within an interdisciplinary team.
  • Handle emergencies without assistance.
  • Assist falling or fallen patients and perform CPR alone.
  • Assess home situation for possible patient abuse.
  • Schedule patient visits independently and update schedule daily.
  • Regularly handle confidential information.
  • Have regular contact with agency staff members, physicians, hospital or medical center staff, and the general public.

Qualifications

Required

  • Prefer one year experience in a related field.
  • Current LPN license in the state of Missouri.
  • Must be a licensed driver.
  • BLS CPR Certification through American Heart Association is required.
  • Ability to provide good written documentation in a timely manner.
  • Flexibility and cooperativeness in fulfilling role obligations.
  • Ability to work within an interdisciplinary team.
  • Ability to handle emergencies without assistance.
  • Ability to assist falling or fallen patients and perform CPR alone.

Effects or errors in determining patient condition or administering medication could be critical. Employee is frequently to continuously alone with patient and must be able to handle emergencies without assistance. Employee must be able to assist falling or fallen patient; must be able to perform CPR alone.Individual homes may present poor lighting situations, exposure to pets and other allergens, pest infestations and poor cleanliness. Employee may be exposed to irate and abusive patients and families. Employee must be able to assess home situation for possible patient abuse. Employee may be required to drive at night and/or in hazardous conditions; must be able tofollow written/verbal instructions and read a map. Employee must independently schedulepatient visits in timely and efficient manner; checking in with office daily to update schedule. Employee has regular contact with agency staff members, physicians, hospital or medical center staff, general public. Employee regularly handles confidential information.
