Care Manager, RN

Title: Care Manager, RN – Contract to Hire (USC Only)
Location: Must reside in Delaware, preferably near New Castle County.
Travel: Face-to-face visits required; 2-3 days per week travel within the state.

Job Responsibilities:

Conduct face-to-face assessments in members’ homes, nursing facilities, and community settings, with follow-up telephonic contact.
Develop and implement care plans for members with chronic and complex needs in various settings.
Coordinate care across physical, behavioral, and social services, ensuring cost-effective and safe care delivery.
Facilitate care transitions, educate members, and provide resources to meet their goals.
Collaborate with healthcare teams to address members’ needs and ensure service plan effectiveness.
Document all services and interventions in the electronic health record.
Perform other related duties as assigned.

Qualifications:

Registered Nurse in DE with 5+ years in Intensive Case Management and Discharge Planning.
Proficient in assessments, care planning, and collaboration with healthcare providers.
Experience with DME equipment, social determinants, and discharging from facilities.
Must have reliable transportation, a dedicated home workspace, and meet regulatory deadlines.
Proficient in PC-based documentation (Word, Excel, etc.) and electronic health records.

Preferred Qualifications:

CCM, LBSW, LMSW, LCSW certifications.
Experience with HIV/AIDS, behavioral health, developmental disabilities, Medicare, and Medicaid populations.
