Position Overview:
Rehabilitation Associates is at the forefront of revolutionizing post-acute care in Maine, focusing on an interdisciplinary team approach. We are seeking a Medical Assistant to play a crucial supportive role in our Chronic Care Management (CCM) program. As a Medical Assistant in our CCM program, you will help support patients in achieving their health goals by completing a structured recording of their health information. You will work closely with healthcare providers to ensure that patient health information is promptly shared and used effectively within and outside the practice. In addition to the CCM program, the position includes administrative tasks to support providers in patient care delivery. This position involves working remotely, with a flexible four or five-day workweek, in a supportive environment that promotes work-life balance.

Primary Job Responsibilities:
  • Patient Interaction and Provider Support: Serve as a liaison between patients and providers, ensuring clear communication and understanding of care plans. Conduct follow-up calls with patients, specialists and other care team members to monitor chronic conditions and adherence to their care plan.
  • Care Coordination: Assist in monitoring and updating Chronic Care Management care plans. Track and document patient progress, including changes in vitals, medications, and lab results.
