Responsibilities

  • Maintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program.
  • Confers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function.
  • Provides input for meetings and attends during absences of the Director Physical Therapy.
  • Maintains statistical records of individuals' treatments.
  • Assists with in-service training programs upon request.
  • Records and updates individual's plan of care in the Inter-disciplinary Kardex.
  • Submits requisition for supplies needed in the treatment to Director Physical Therapy.
  • Assumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation.

Qualifications

Required

  • Graduation from an approved school of physical therapy.
  • State license to practice physical therapy.
  • Ability to direct others.
  • Regular interaction with residents, coworkers, visitors, and/or supervisors requires the ability to read, write, speak, and understand the English language at an intermediate or more advanced level.

Preferred

  • Two years' experience in physical therapy.

We are see king a high quality, engaging Physical Therapist to be on the front lines, making the difference in the lives of our residents and their families. We have positions available forfull time and per diem.

Elderwood at Williamsville is also is home to WNY's only non-hospital-based ventilator unit. The 22-bed unit is staffed by respiratory therapists and trained nurses offering 24-hour ventilator care under medical supervision of a board-certified pulmonologist. This faciltiy features a b right, airy atrium overlooking Bassett Park, spacious patios surrounded by gardens, and a spacious rehab gym with state-of-the-art equipment. We also currently rated 3 Stars by CMS!

Physical Therapist Position Overview:

Physical Therapy team members assist with ensuring the health and well-being of individuals by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality care is maintained at all times. State specific Physical Therapy license is required.

In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
