As an Outpatient Therapist at Sevita, you will provide therapeutic interventions to youth, families, and adults in various settings including homes, communities, and virtually. - Provide mental health consultation to stakeholders and treatment team members.
- Develop individualized treatment plans in coordination with an interdisciplinary team
- Develop and coordinate discharge planning.
- Maintain timely and accurate health record documentation.
- Attend regular staff meetings, clinical meetings and one-on-one sessions with direct supervisor to process current status of cases.
- Provide 25 service delivery hours per week.

Required
- Bachelor's, LSW + at least 3 years' experience
- Master's Degree in human services + LPC, MFT
