Job Description
Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.
• Performs clinical diagnosis in conjunction with conducting a comprehensive bio-psychosocial assessment and establishment of a care plan.
• Champions integrated care model treatment guidelines in the primary care setting by providing approximately 6-8 sessions of short-term, focused treatment.
Qualifications
Master’s in social work or related field required
LCSW, LPC, LMFT, or LADC required
Prior experience working in a medical setting preferred.
Bilingual preferred.
