Overview We are looking for a Remote Care Manager AIM (Advanced Illness Management) Full Time RN to join our team. This position will directly report to the Director of Care Management AIM and is responsible for working with patients, their families, PCP, IDT, and other health care professionals to provide and coordinate high-quality end of life care.
