The Department of Psychiatry & Behavioral Sciences at Montefiore Medical Center is recognized as one of the best in the nation. It fuses together a unique blend of extraordinary physicians, a renowned clinical team, and exceptional educational and research resources to help patients cope with mental health disorders. The Outreach Specialist will assist individuals who qualify for the High-Risk Care Management Program (HRCM). The HRCM program is intended to assist high acuity patients transition from inpatient to outpatient care. The HRCM program is a component of the Behavioral Health Centers of Excellence award.

The Clinical Manager manages and directs clinical and administrative activities for the Department of Psychiatry and Behavioral Sciences and The High-Risk Care Management Team.
Oversight of intensive care management team delivering services to eligible high risk individuals following discharge from MH/SUD inpatient/ER in the month following discharge, and the 3 months following discharge.
