  • Develop, coordinate, and continually improve processes pertaining to the Service Line coordination of care.
  • Serve as a resource and consultant for the Service Line as well as the community, developing a presence in the professional and general public.
  • Facilitates referrals by collaborating with referring providers

Qualifications

Required

  • BSN or Master's Degree
  • 5 years nursing experience
  • Experience within the Service Line
  • Experience in a multi-specialty environment
  • Experience with home care/wound care ideal
  • Licensed as a professional RN in the state of Connecticut

Job Description

The Nurse Navigator plans, coordinates and delivers health services to patients receiving care within the defined Service Line. This includes working collaboratively with other health providers to assure coordinated care throughout the service line’s continuum of care. The nurse navigator will assist with the coordination of services and will provide support and education throughout diagnosis and treatment for patients. She / he will serve as a professional resource in the areas of the Service Line.
