  • Provides crisis intervention to patients and families as needed.
  • Functions as a patient advocate and as a liaison between the patient and multiple care delivery systems.
  • Participates in comprehensive inter-disciplinary team patient care meetings with appropriate documentation on IDT assessment and plan of care.
  • Records psychosocial assessments, update assessments and progress notes in accordance with established recording guidelines.

Required

  • LMSW or LLMSW License
  • LLMSW required. Must obtain full licensure upon 12 months of completing 4,000 supervised hours in a full time status. A limited licensed MSW must practice under direct supervision of a LMSW. A LLMSW must sit for and pass The State Board after acquiring 4,000 hours of supervised social work experience. This previous supervised work experience may be acquired outside of the Henry Ford Health System.

Full Job Description

Unit: Fairlane Dialysis ( CA/CC & HHD)

Fairlane Home Dialysis is a unit attached to an outpatient dialysis facility .  The facility oversees peritoneal and home dialysis patients. Census is 13 PD patients and 2 HHD patients.

Staffed with full-time Nephrologists.  In addition, we have full-time and part time Home Nurses and home dialysis technicians. In addition, the unit has multiple support staff members that are shared with the outpatient clinic including front desk team and tech leader staff ( machine maintenance) that may assist with operational duties within the clinic.

We case manage 15 pts who are monitored by connected health for Baxter and NxStage systems. Patients are trained to perform dialysis treatments at home within the clinic. PD training takes 7-10 days and HHD training takes 4-6 weeks. The home team schedules monthly clinic visits for all home patients with the interdisciplinary team and lab draws for PD patients. Our team provides pt education as well as referrals to transplant services for sites the pt requests to be referred to.

The unit is typically open Monday through Friday 8am to 430pm for pt care hours with some flexibility based on the needs of the clinic. Home Nurses are scheduled to take daily after hour on-call including weekends. The home clinic is closed most weekends and holidays depending on the needs of the clinic ( training or clinical need for a patient ).

WHAT WE ARE LOOKING FOR:

Masters-prepared Social Workers to work with our renal patients and families to assist with the adjustment to what is likely a life-changing event. Provide emotional and social adjustment counseling, assistance with removing barriers to treatment, education on kidney disease and treatment goals, and bridging barriers to referral services and community resources.

+ Conducts initial psychosocial interviews with patients and families in order to assess baseline level of functioning, comprehension of illness and its ramifications, and impact of individual/family history upon current and future psychosocial functioning.
