  • Maintain current healthcare information on the ICF Residential and Day Program participants in the agency's Electronic Medical Records
  • Collaborates as part of the interdisciplinary team in assessing, planning, implementing and evaluating programs and services for individuals
  • Advocate, work, train and support individuals, Arc staff, families and Hospice staff on individuals that are on Hospice services at home
  • Involved with coordinating, possibly scheduling, and/or attending appointments
  • Directly supervises Approved Medication Administration Personnel (AMAP) with respect to the medication administration
  • Cover/Perform Medication/Insulin Administration, Tube feeds, Colostomy Care and/or Wound care in support of staffing needs
  • Resource as the point professional related to Medical Orders of Life Sustaining Treatment (MOLST: DNR/DNI) and Hospice
  • Oversee LPN's in coordination and overseeing the health care needs for the individuals served within their caseload

Qualifications

Required

Description

We are seeking a full-time Registered Nurse to provide nursing services and medical management to individuals with developmental and intellectual disabilities at our Palmer Center, an Intermediate Care Facility (ICF) in Kingston.

This position reports to the Senior RN on-site. Responsibilities include, but may not be limited to, the following:

+ On-site Nursing for ICF Residential and Day Program needs

+ Assessing and Triaging symptoms, issues and concerns daily for individuals with multiple co-morbidities.

+ Provide nursing care and/or direction for minor illnesses, emergencies, or injuries.

+ Compose individually specific nursing plans of care, per agency protocols, to support staff.

+ Collaborates as part of the interdisciplinary team in assessing, planning, implementing and evaluating programs and services for individuals.

+ Advocate, work, train and support individuals, Arc staff, families and Hospice staff on individuals that are on Hospice services at home.

+ Involved with coordinating, possibly scheduling, and/or attending appointments (routine and/or emergent). Will review, communicate and follow up on needs as ordered and maintain collaboration with direct supervisor and the interdisciplinary team as appropriate.

+ Directly supervises Approved Medication Administration Personnel (AMAP) with respect to the medication administration and assures AMAP's remain current with their certifications and related policies and procedures. This includes Tube feeding, Insulin certifications and Colostomy Care as appropriate along with related policies and procedures.

+ Cover/Perform Medication/Insulin Administration, Tube feeds, Colostomy Care and/or Wound care in support of staffing needs as maybe needed and directed by supervisor.

+ Resource as the point professional related to Medical Orders of Life Sustaining Treatment (MOLST: DNR/DNI) and Hospice to support of individuals at the end of their lives. This includes collaboration with parties such as: primary care providers, hospital doctors/social workers, families, teams, Mental Health Legal Services (MHLS) and Hospice.

+ Oversee LPN's in coordination and overseeing the health care needs for the individuals served within their caseload. This oversight shall be in accordance with applicable Federal and State regulations.

HOURS: 40/Flexible

STARTING SALARY: $ 74,000 to $ 81,400 per year

Minimum Requirements

+ NYS Registered Nurse license

+ One year of clinical experience

+ OPWDD and I/DD experience a plus

+ Computer skills
