The Admissions Supervisor at Community Hospice of Northeast Florida is responsible for collaborating with Admissions Managers to achieve department goals, coordinating, directing, organizing, and supervising the Admissions Team. Responsibilities

  • Informs Director of Admissions regarding active referral status, risk management issues, treatment modalities and admission advancement status per hospital.
  • Works with the Customer Experience Center (CXC) department staff to assure referral information is transmitted accurately and timely to the Admissions staff.
  • Provides prompt, same day, quality service recovery to an account when required.
  • Functions as a positive role model/leader and interdependent team member. Assists the department director in monitoring and reporting quality improvement and corporate compliance initiatives.
  • Works to ensure compliance with documentation.
  • Demonstrates ability to use critical thinking skills to solve problems independently.
  • Develops and continually grows positive professional relationships with referral sources.
  • Demonstrate knowledge and ensure compliance to state and federal guidelines for Hospice program.
  • Attends a minimum of 85% of the general staff and team meetings. Promotes agency image within the community.
  • Practices and promotes positive peer and patient relations. Develops areas of expertise in supervision and hospice nursing skills.
  • Pursues professional growth through academic and continuing education programs.
  • Adheres to all internal controls.

Qualifications

Required

  • Current licensure as registered nurse in State of Florida.
  • Current State of Florida driver's license with driving record acceptable to Community Hospice insurance carrier.
  • Possession of a Bachelor's degree in health related field.
  • Healthcare related certification.
  • Minimum of three (3) years Hospice Admissions
  • Knowledge of customer services and marketing techniques.
  • Competent in computer programs and reporting.
  • Ability to multitask.
  • Must be flexible and adaptable to change, work prioritization and stress management.
  • Effective written and oral communications

Under the supervision of the Director of Admissions, the Admissions Supervisor, is responsible for collaborating with Admissions Managers to achieve department goals; as well as will be responsible for coordination, direction, organization and supervision of the Admissions Team. Facilitates speed to care by providing a range of preadmission and admission services. Collaborates with internal and community partners to resolve barriers to the Community Hospice admission. Provides education regarding Community Hospice services and philosophy. Collaborates with the manager, physicians, nurses and case managers to ensure the best outcome for patient, families and referral sources. Contributes to the smooth function of services, advocating for patient and family needs, coordinates care and facilitates transfer of care to appropriate team. Has an understanding of Medicare Conditions of Participation for Hospice, how insurances pay for Hospice and State Regulations that apply to Hospice admission. Provides education and information to referral sources, physicians, patients, and their families. Represents the organization in a positive professional manner understanding the importance of the first impression.

Primary Responsibilities:

Required Licenses/Certifications:

Education/Experience:

Any combination of education and experience that would provide required skill and knowledge for successful performance would be qualifying.

Typical qualifications would be equivalent to:

• Possession of a Bachelor's degree in health related field.

• Healthcare related certification.

• Minimum of three (3) years Hospice Admissions

Knowledge of:

• Customer Services and marketing techniques.
