- Monitors time, charting and statistics of social work staff in accordance with departmental criteria; participates in interdisciplinary treatment planning conferences.
- Performs psychosocial assessments of patients and families; identifies patients needing psychosocial services.
- Performs competency assessment/performance appraisals according to institutional schedules and policies.
- Interviews patients and families to assess family structure, dynamics, adjustments to illness and access to necessary resources; identifies medical problems and predicts course.
- Educates patients and families about community resources and discharge and planning procedures.
- Participates in internal and external committees and workgroups that require social work expertise and best practice input.
Qualifications
Required
- Master of Social Work
- Current License to practice as a Clinical Social Worker in New York State
- 4-6 years of relevant experience
- 0-2 years of leadership / management experience
