  • Monitors time, charting and statistics of social work staff in accordance with departmental criteria; participates in interdisciplinary treatment planning conferences.
  • Performs psychosocial assessments of patients and families; identifies patients needing psychosocial services.
  • Performs competency assessment/performance appraisals according to institutional schedules and policies.
  • Interviews patients and families to assess family structure, dynamics, adjustments to illness and access to necessary resources; identifies medical problems and predicts course.
  • Educates patients and families about community resources and discharge and planning procedures.
  • Participates in internal and external committees and workgroups that require social work expertise and best practice input.

Qualifications

Required

  • Master of Social Work
  • Current License to practice as a Clinical Social Worker in New York State
  • 4-6 years of relevant experience
  • 0-2 years of leadership / management experience

Job Description

Job Qualification

+ Master of Social Work required.

+ 4-6 years of relevant experience and 0-2 years of leadership / management experience, required.
