  • Develop plans of care, stratify risk levels, and address patient needs and conditions.
  • Identify the appropriate level of care, apply utilization management methodologies, monitor utilization and high-cost resources.
  • Collaborate with RNs to manage critical values, crisis situations, and escalate health concerns.
  • Engage and enroll new patients into the ECM program.
  • Collaborate with team members to create and deliver systematic case reviews.
  • Maintain working knowledge of insurance, county, and community-based resources.
  • Develop and update department policies, procedures, and guidelines.
  • Home and/or community-based visits may be necessary.

Qualifications

Required

  • High school diploma or equivalent.
  • Graduate of an accredited LVN program.
  • Associate degree preferred.
  • Current California licensed vocational nurse (LVN).
  • Current CPR/BLS certification.
  • Valid California driver's license and auto insurance.
  • 3+ years of case management, skilled nursing, out-patient clinic, or equivalent experience.
  • Proficiency in MS Office Suite.
  • Ability to use widely supported internet browsers and various electronic health records.

What We Are Looking For

The CMT Care Manager, LVN (LVNCM) works collaboratively within an interdisciplinary care team to improve the health and well-being of patients enrolled in the Care Management Team. This position will work with a high-risk population of patients, addressing a wide spectrum of health concerns including, but not limited to, homelessness, severe mental illness, substance use disorders, social determinants of health, and individuals' post-incarceration. Maintains working knowledge of specialized case and utilization management methodologies and practices; applies concepts to everyday practice. The practice of Care Management will include the outreach and engagement of patients, facilitating transitions of care, linkage to resources across organizations and the community at large, coordination of care, and ensuring the appropriate utilization of care for patients. This position will function under the clinical supervision of a Registered Nurse.

Schedule: 5 days per week, 8 hours per day, Monday - Friday | Location: San Bernardino, CA

ESSENTIAL FUNCTIONS AND DELIVERABLES
  • Engage and manage a caseload of 50-70 moderate to high-acuity pediatric patients while also overseeing clinical coordination of patient services, linkage across organizations and services, medical collaboration, and care review.
  • Identify patient needs across the healthcare spectrum, including a comprehensive review of medical, behavioral, and social needs, and collaborate with providers and healthcare team members across the healthcare continuum to ensure patient needs are comprehensively communicated across healthcare organizations and payers, and to negotiate care gaps and resolve issues arising during coordination of care.
  • Collaborate with the RN to develop plans of care, stratify risk levels, and address patient needs and conditions.
  • Identify the appropriate level of care, apply utilization management methodologies, monitor utilization and high-cost resources, provide feedback, and collaborate with the team to identify cost-efficient options.
  • Collaborate with RNs to manage critical values, crisis situations, and escalate health concerns appropriately, while also evaluating and prioritizing care options and resources.
  • Engage and enroll new patients into the ECM program while assisting with eligibility and patient recruitment, completing screenings, and monitoring benefits periods.
  • Collaborate with team members to create and deliver systematic case reviews.
  • Maintain working knowledge of insurance, county, and community-based resources to aid in continuity of care.
  • Develop and continuously update department policies, procedures, and guidelines to ensure they remain aligned with program changes and contractual updates.
  • Home and/or community-based visits may be necessary based on patient and team need; must have a reliable vehicle, valid driver's license, and auto insurance.

QUALIFICATIONS:
  • Education: A high school diploma or equivalent is required. Graduate of an accredited LVN program. An associate degree is preferred.
  • Licensure/Certification: Current California licensed vocational nurse (LVN), current CPR/BLS certification (must be American Heart Association or Red Cross accredited program). Valid California driver's license, and auto insurance. As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
  • Experience: 3+ years of case management, skilled nursing, out-patient clinic, or equivalent required.
  • Essential Technical/Motor Skills: Must be proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook). Must be able to use widely support internet browsers. Must have the ability to use variations of electronic health records and other various databases.
  • Interpersonal Skills: Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English in person, in writing, and on the telephone; think critically; perform basic math functions; manage multiple assignments effectively; compose written material; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision.
  • Essential Mental Abilities: Must be flexible to perform a variety of tasks. Must be well organized and a self-starter. Must have strong analytical and problem-solving skills and the ability to prioritize tasks efficiently in a fast-paced healthcare environment.
