- Facilitate discharge planning in line with goals for length of stays related to difficult to place, high-cost, and escalated patients.
- Use a multi-step escalation protocol to identify patients with barriers that prevent effective discharge within the goals for length of stay.
- Work with the interdisciplinary team to assume responsibility for escalated patients according to set standards for discharge planning.
- Monitor difficult to place and high-cost patients to identify early intervention opportunities for timely and cost-saving discharges within accepted utilization review guidelines.
- Gather pertinent information utilizing a broad scope of sources to draw conclusions, determine needs, and take actions to meet those needs.
- Oversee the efficient and effective management of care and appropriate utilization of resources.
- Work collaboratively with members of the multi-disciplinary team to develop and implement a plan of care.
- Communicate clearly and effectively with patients, care team members, and community partners.
- Provide leadership and creativity for programs and projects to enhance the effectiveness of care management.
Qualifications
Required
- Master's degree in a relevant field.
- Current/Valid license in the State of Maryland (LCSW-C/LCPC).
- Minimum of 3 years of experience.
Preferred
- 5+ years of experience.

JOB SUMMARY:
To facilitate discharge planning in line with goals for length of stays related to difficult to place, high-cost and escalated patients. Works collaboratively with members of the multi-disciplinary team to develop and implement a plan of
care.
Consistently demonstrates the ability to communicate clearly and effectively with the patients, care
team members and community partners.
Provides leadership and creativity for programs and projects to enhance the effectiveness of care
management.
REQUIREMENTS:
Seasoned professional knowledge; equivalent to a Master's degree; knowledge
in more than one discipline.
Current/Valid license in the State of Maryland; LCSW-C/LCPC required.
Minimum of 3 years of experience; 5+ years of experience preferred.
