Responsibilities

  • Complete Medicare Annual Wellness visits with eligible patients
  • Assist with clinic nursing responsibilities as defined or needed
  • Ensure understanding of CMS Medicare Annual Wellness Visit requirements/visit workflows
  • Provide and facilitate competent health care of patients and families in area of practice
  • Ensure adherence to established standards of care and scope of practice
  • Ensure documentation of care is complete and accurate
  • Provide health and wellness education to individuals and groups
  • Act as preceptor and support the development of other staff and healthcare students
  • Provide leadership for initiatives designed to improve the quality of care or clinical outcomes
  • Assist Nursing Supervisor/coordinator in the development of departmental standing orders
  • Work with Nursing Supervisor to ensure department is compliant with DNV standards
  • Perform telephone visits with eligible Annual Wellness Visit patients
  • Complete AWV as assigned per departmental benchmarks/guidelines
  • Assist with coordination of care for patients age 18-150
  • Perform telephone triage of both acute & chronic needs for patients age 18-150
  • Assist with in office procedures, such as incision & drainage, joint injections
  • Administration of immunizations & medications to patients age 18-150

Qualifications

Required

  • Registered Nurse license as defined by the Illinois Board of Nursing

The Ambulatory Nurse utilizes a systematic approach to nursing by incorporating assessment, planning, implementation and evaluation in the practice of nursing.

Provides healthcare to individuals, families, and communities within the RN scope of practice as defined by the Illinois Board of Nursing.
