  • Strong organizational skills, passion and compassion for the population
  • Ability to work cooperatively with multidisciplinary team

We have our eye out for a candidate with a proven track record of successfully coordinating services for residents, while remaining committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with a positive and supportive team of healthcare professionals.

Job Responsibilities:

+ Maintain firm adherence to the resident's "Bill of Rights", as they relate to the facility

+ To communicate with the resident's care providers

+ Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals

+ Coordinate and supervise clinical care for residents

+ Maintain clear and concise documentation in accordance with facility requirements
