Baptist Senior Family is seeking a dedicated Service Coordinator for the independent living department.   • Education-Attend seminars and network with other service coordinators organizations & social service agents; 36 hours of training are required in the first year and 12 hours of training ever year thereafter
  • Network with social service agencies to build contacts and create a directory of community resources and services that is available in a central location for residents and HUD inspection
  • Collaborate with service providers to provide services based on resident needs and interests
  • Inform residents on new and continuing community resources and maintain the resource center with updated information
  • Conduct health & wellness assessments at initial move-in, annually and as needed
  • Develop resident care plans based on results of health & wellness assessments, follow up with residents at initial move-in, annually and as needed
  • Identify residents with unmet needs and facilitate access to services and entitlement benefits and/or develop new resources as needed
  • Monthly newsletter - summary of events
  • Health & Wellness events-blood pressure screenings, podiatrist visits, etc.
  • Parking Lot Assignments & Vehicles - Prepare application form and review resident's current registration, insurance card and inspection sticker and track annually. Prepare parking violation notices after 30 days from the date of letter to resident if any items are non-compliant. Assign parking permits and parking spaces to residents upon initial registration of their vehicle or if vehicle is replaced. Retrieve parking permits when resident moves out.
