The Providence Center Adult & Children Clinical Intake Specialist is responsible for assessing and evaluating the mental health and/or substance use conditions of new or returning adult or child/adolescent clients to determine if services are needed and to link them to the most appropriate services.   • Perform record keeping in accordance with Health Information Services and other Center requirements.
  • Collaborate with other providers, agencies, and individuals in the clients' network of care.
  • Attend trainings, case presentations and conferences.
  • Maintain cooperative relationships with Center staff, clients, community agencies and the public.
  • Maintain knowledge and familiarity of all adult & child programs within The Providence Center, in order to appropriately assign clients.
  • Communicate with various local agencies and other departments or teams at The Providence Center.
  • Serve on appropriate TPC committees.

Qualifications

Required

  • Master's degree in social work and/or relevant clinical experience.
  • Experience completing clinical assessments preferred.
  • LICSW/LMHC preferred. Licensure eligibility required.
  • Bilingual Spanish preferred.
  • Ability to write reports and correspondence.
  • Ability to speak effectively with clients, community agencies and Center employees.
  • Insured auto and valid driver's license.

Job Summary:

The Providence Center Adult & Children Clinical Intake Specialist will assess and evaluate the type and degree of a new (or returning) adult or child/adolescent client's mental health and/or substance use condition to determine whether services are needed and to link the client to the most appropriate services.

Establish a psychiatric diagnosis, collect data sufficient to permit a case formulation and to develop an initial treatment plan as needed.

Duties and Responsibilities:
