  • Provide outreach for all patients due for follow up and new patient appointments according to policy and procedures.
  • Provide all applicable patients with IMM and Moon forms as required by regulation.
  • Document all correspondence in electronic medical records as necessary.
  • Liaison to multiple departments, agencies and provider offices.
  • Coordinate transportation for all patients as needed
  • Answers phones courteously and takes messages completely and accurately.
  • Data tracking
  • Prepare and deliver routine correspondence/medical records to referring agencies and primary care providers in accordance with policy.
  • Aid with insurance as needed, including helping patient connect with resources for enrollment, conducting verification of insurance plans, assisting patients with re-determination, and others as needed.
  • Follows all safety precautions for patients: patient identification by the use of two patient identifiers.

Qualifications

Required

  • High School degree or equivalent required
  • Associates degree preferred
  • Excellent oral communications to solicit information and communicate effectively
  • Knowledge of Microsoft Office
  • Written, verbal and exceptional interpersonal communication skills
  • Prefer knowledge of medical terminology
  • Ability to multitask
  • Ability to function independently, to organize, and be self-motivated

Full Job Description

Position Summary: The Patient Care Navigator is responsible for assisting nurses and social workers to provide outreach efforts to a group of patients from a patient list. The Patient Navigator will share knowledge and education to empower patients and improve their ability to navigate services and systems. This role will utilize the established model of care management. The individual who holds this position exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures, including complying with all Rush University Medical Center Customer Service Standards.

Position Performance Area/Expectations:
Care Management Navigation
