  • Contact patient to cancel and reschedule appointments as needed and call to patients who do not keep their appointment, noting it in practice management system and reporting no shows to referring providers
  • Contribute to provider and clinic efficiency by coordinating and scheduling surgeries.
  • Contact specialist or other health care facilities to obtain procedure reports of referred patients
  • Maintain a cooperative relationship among health care teams by compiling statistics, communicating information, responding to requests, building rapport and participating in problem-solving methods
  • Refer patient to Outreach department if eligibility for sliding fee is identified or if a change in payor source is identified
  • Run reports monthly to confirm patient appointments

  • Experience as a receptionist in a health care setting preferred

Description

STATEMENT OF PURPOSE:

The Care Coordinator facilitates all aspects of the patient referral process. When Sun Life medical providers refer patients to external providers for additional medical care and services, the Care Coordinator completes all steps necessary to arrange the medical care and services for the patient, and to obtain procedure reports from the external providers when referred care and services are completed.

ESSENTIAL FUNCTIONS:
  • Obtain authorization for care by contacting the appropriate insurance carrier, communicating the providers instructions, and transmitting information as needed to obtain authorization. Contact patient upon receiving authorization.
  • Prepare referrals for all tests and procedures for patients, make patient appointments, and transmit required documentation.
  • Maintain documentation of patient services by completing patient and referral records in accordance with established policies and procedures
  • Input referral data into the practice management system and maintain a log of referrals made
  • Contact patient to cancel and reschedule appointments as needed and call to patients who do not keep their appointment, noting it in practice management system and reporting no shows to referring providers
  • Contribute to provider and clinic efficiency by coordinating and scheduling surgeries, i.e. surgeon, assisting surgeon, hospital facility, primary care provider; notifying appointment desk to block providers time; and by distributing applicable documentation and information to outside facilities
  • Contact specialist or other health care facilities to obtain procedure reports of referred patients
  • Maintain a cooperative relationship among health care teams by compiling statistics, communicating information, responding to requests, building rapport and participating in problem-solving methods
  • Refer patient to Outreach department if eligibility for sliding fee is identified or if a change in payor source is identified
