What We're Looking For:

• A current Nurse Practitioner license in Michigan.
• Master's degree in Nursing with advanced skills in physical assessment.
• Minimum of 3 years of clinical nursing experience, with at least 1 year preferred in hospice and palliative care.
• Strong communication, collaboration, and interpersonal skills.
• Proficiency in electronic medical records and other healthcare technology.
• A commitment to patient-centered care and the Community mission.
