Responsibilities

• Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients.
• Perform duties and standards according to established policies, procedures, and competency guidelines within the department and organization.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within patients capabilities.
• Participate with all other home care personnel in patient care planning.
• Take initial history and make initial evaluation.
• Consult with physicians regarding patients progress
• Participate in quality assessment and performance improvement activities as assigned.
• Prepare clinical and progress notes.
• Help develop the plan of care and revise, as necessary.
• Consult with family and agency personnel.
• Ability to do extensive bending, lifting and standing on a regular basis.

Qualifications

Required

• Graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or Association
• Active license to practice Physical Therapy in MN
• Minimum of four (4) years experience as a Physical Therapist
• Must have criminal background check
• Must have current CPR certification
• Must have active driver's license and clear driving record
• Computer skills
