  • Treat the participant through the use of therapeutic activities designed to restore function and self-care activities for the purpose of improving function
  • Observe, record, and report the participant's reaction to treatment and any changes in the participant’s condition to the physician, PACE Clinical Director or IDT as appropriate

Qualifications

Required

  • Registered by the American Occupational Therapy Association or successfully completed the educational requirements of occupational therapy
  • State of Washington Occupational Therapist license
  • Health Care Provider level CPR certification
  • Minimum one (1) year of occupational therapy experience

Preferred

  • Minimum one (1) year of experience working with frail or elderly population
  • Experience in Medicare Certified Home Health

The Home Care Coordinator works under the general supervision of the Clinical Director and is responsible for organizing the plan of care for participants while providing education and eliminating any potential barriers to treatment. The Home Care Coordinator monitors the quality of care and services being provided to program participants in the home. Ensures continuity of home care service delivery at the time of enrollment and throughout participant’s members as well as compliance with all governing regulations and agency policies and procedures.

However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
