  • Oversee internal and external referrals for healthcare services and specialists.
  • Promote cohesive and effective team dynamics.
  • Manage direct admit to skilled nursing facility (SNF) process.
  • Participate in the interview process and collaborate on team member hiring.
  • Complete performance reviews, identify individual goals, mentor and support team members.
  • Collaborate to develop innovative solutions and programs to overcome healthcare barriers.
  • Work closely with market leaders to facilitate team member education and onboarding.
  • Perform other related responsibilities as assigned.

Qualifications

Required

  • Reliable access to high-speed internet for remote work communication and productivity.
  • Degree from an accredited nursing program with a minimum of an Associate or Diploma in Nursing.
  • Active, unrestricted Registered Nurse license.
  • 2+ years of continuous hospital clinical experience within the past 10 years.
  • 2+ years of continuous ambulatory clinical experience within the past 5 years, preferably in care management or practice management.
  • Experience using data to drive strategy and execution.
  • Skilled using the Microsoft Office suite of products.
  • Experience in interdisciplinary team management.
  • Ability to arrange travel to various work sites as needed.
  • Possess and maintain a valid driver's license in state of residence and motor vehicle insurance.

Preferred

  • Bachelor's of Science in Nursing.

Your Role

The Clinical Operations Supervisor, RN will be responsible for evaluating and supervising overall activities for health maintenance and promotion programs. The Clinical Operations Supervisor, RN will provide daily leadership and be accountable for clinical programs for product lines and members served to ensure contractual compliance and achievement of clinical, quality, and utilization management goals. The Clinical Supervisor, RN will be responsible for developing the capabilities and refining the processes and workflows required to effectively grow and manage the Clinical Operations team and facilitate the successful implementation of clinical programs, vendor relationships, and technology enabled health programs. The Clinical Operations Supervisor, RN will work closely with the Market Operations team and report to the Director of Nursing and Clinical Operations. This role oversees a team that includes Nurse Care Managers and will work closely with Social Workers, Care Coordinators, and Transition of Care Coordinators. This position will partner with physician leaders to ensure the overall success of the market and team operations.

This position is remote, but you must be located within or nearby our Kalamazoo market to ensure availability to meet with team members in person as necessary, as well as to visit market-based physician offices and health care facilities as needed.

Primary Functions Include:

  • Provide direct and indirect oversight of inter-disciplinary care teams: Nurse Care Managers (direct), Social Workers, Care Coordinators, Transition of Care Coordinators, etc.
  • Monitor clinical and business dashboards/metrics that drive patient outcomes and performance metrics as it relates to their team's performance.
  • Understand HEDIS measures and standards. Assist in the development of a plan to close gaps, as needed.
  • Manage Complaints and Grievances, ensuring required response timeframes are met. Direct service recovery to promote optimal patient & partner satisfaction.
  • Oversee processes to manage internal and external referrals for healthcare service (e.g., behavioral health (BH) referral, home health) and specialists (e.g., cardiology, pulmonology).
  • Promote cohesive and effective team dynamics to ensure success leveraging multiple key performance indicators.
  • Manage direct admit to skilled nursing facility (SNF) process with your team by ensuring appropriate criteria and medical documentation is submitted to accepting agency. Work in collaboration with medical team, offices, medical records.
  • Participate in the interview process & collaborate with recruiters on team member hiring.
  • Team member oversite: Complete performance reviews, identify individual goals, mentor and support team members for professional growth and learning opportunities.
  • Collaborate to develop innovative solutions and programs to overcome healthcare barriers
  • Work closely with market leaders to facilitate team member education and onboarding to ensure. education and training is completed. Identify team member needs: knowledge, skills, and abilities.
  • Perform other related responsibilities as assigned.

The base pay range for this role is $94,300 - $108,400. Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, and organizational needs. Base pay is just one piece of the total rewards program offered by Honest. Eligible roles also qualify for short-term incentives and a comprehensive benefits package.
