Qualifications

Required

  • Bachelor’s Degree from an approved School of Nursing.
  • Licensed in Maryland as a Registered Nurse.
  • Personal vehicle and valid Maryland driver’s license.
  • Demonstrate comfort working with individuals who are homeless and/or low-income.
  • Possess excellent verbal and written communication skills.
  • Demonstrate comfort independently delivering clinical nursing services primarily outside the medical clinic.
  • Willingness to integrate principles into practice such as Harm Reduction, Motivational Interviewing, and Housing First.
  • Strong organizational and time management skills.
  • Able to cope with interruptions and be a team player.
  • Flexible approach, working with several cross-disciplinary teams in a collaborative style.
  • Approaches change with a positive, open-minded attitude.
  • Able to work with client who have significant mental health challenges, manage crisis situations and assess for safety.
  • Able to work with ill, disabled, emotionally upset, and sometimes hostile clients.

Preferred

  • Two years of case management experience.
  • Two years of clinical nursing experience.
  • Experience working with individuals who have behavioral health disorders.

Overview

As a member of the Housing Services team, the RN Case Manager has direct clinical care responsibilities. The Housing Services program was developed for the purpose of helping individuals with behavioral health disorders maintain housing placements. The RN Case Manager’s role is to provide medically-related case management services to clients served by the various Housing Services programs. Within the scope of the RN licensure, the RN will act as a conduit to clients’ medical provider, provide nursing support, and collaborate with the multidisciplinary team in order to improve overall function and produce better housing placement outcomes.

Key Role Responsibilities

  • Provide nurse case management services to clients in the Housing Services program to include medical care coordination, linkage to health related resources, monitoring referral outcomes and developing productive working relationships with the local medical community.
  • Deliver health education and counseling, drawing upon the individual’s strengths, resiliency and motivation, in order to explore lifestyle choices, preferences, and safety.
  • Perform clinical tasks including nursing assessments, nursing treatments, medication administration and adherence support.
  • Performs home visits, community visits and escorts independently and in accordance with clients’ plan of care
  • Demonstrate positive regard and respect for clients through speech and actions, utilizing a client-centered, non-judgmental, trauma informed approach.
  • Involve the client in the development and implementation of an integrated treatment plan using SMART goals.
  • Complete documentation within the client’s electronic health record in a manner that is easy to understand and in accordance with established formats and required timeframes. Documentation includes but is not limited to: Initial assessments, treatment plans and progress notes

Key Agency Responsibilities

In addition to role responsibilities, each staff member of Health Care for the Homeless has the following responsibilities as a part of their employment:

  • Models and reinforces the Health Care for the Homeless “core values” of dignity, authenticity, hope, justice, passion and balance
  • Actively participates in performance improvement activities and actively participates in advocacy activities that support the mission of Health Care for the Homeless
  • Performs other duties on an as-needed basis
  • Protects our client’s personal health information by maintaining compliance with HIPAA and other relevant Health Care related IT security regulations

Knowledge, Experience, and Skills

Formal Education and Training

  • Bachelor’s Degree from an approved School of Nursing.
  • Licensed in Maryland as a Registered Nurse

Experience

  • Two years of case management experience preferred
  • Two years of clinical nursing experience
  • Experience working with individuals who have behavioral health disorders preferred
