Position Summary

The Homecare Nurse Aide plays a critical role in assisting participants in a way that supports and maintains the health and function of the participant which is critical to supporting their desire to remain at home. The Homecare Nurse Aide will assist with providing personal care, meal preparation, light housekeeping tasks, and transportation that is outline in the life plan that is individualized per each participant by the Interdisciplinary Team at LifeCircles. Other duties that are include are care support will be provided primarily in the home and occasionally at the LifeCircles Day Center, will assist with lifting, turning, moving, positioning, and transporting participants as needed, utilizing good body mechanics and safety precautions, including the appropriate/proper use of assistive devices and mechanical lifts. Homecare Nurse Aide will observe participants for changes in emotional, behavioral, and physical condition and assure the required documentation related to the participant’s plan of care is accurate and complete per LifeCircles PACE policies
