Patient:

- Needs help with day-to-day activities

- Meets the Indiana Aged and Disabled Waiver requirements
Caregiver:
- Must be at least at 18 years of age
- Eligible to work in the United States
- Not exclude to work for any Medicaid or Medicare Provider
- Does not have a Felony in the last 10 years
- Able to pass a urine drug screen
* The Caregiver may be a non-family member or a family member who is able to provide the care in the patients home.
