Responsibilities

  • Meet with new admissions and their representatives promptly; provide orientation to facility, complete psycho social's, develop comprehensive care plans.
  • Provide direct social work services and counseling to residents and families, individually or in a group setting as appropriate.
  • Complete all documentation according to the Policies and Procedures of the Department of Social Services following the guidelines put forth by NYSDOH/CMC.
  • Coordinate and attend weekly Interdisciplinary Comprehensive Care Plan meetings, discharge planning meetings, weekly staff meetings and mandatory in-services.
  • Complete MDS+, PASARR SCREEN, PHQ 2-9's and other evaluations as required by regulation.
  • Contribute to departmental performance improve projects.

Qualifications

Required

  • MSW from an accredited School of Social Work.
  • Eligible to be registered as a Licensed Masters Social Worker (LMSW) with the New York State Education Department.
  • Minimum one year post graduate in a hospital or long term care setting preferred.
  • Experience in casework, group work and family therapy.

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY. The Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin's Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our Independent Living community (Fountaingate Gardens) and our Certified and Licensed Home Care Service Agencies.

This position is Monday - Friday 8:30 - 5:00.

RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:

+ Candidate would perform social work duties which include but are not limited to discharge planning, helping the resident and/or their representatives with social, emotional, economic and rehabilitation concerns.

+ The assigned caseload will include both long-term and short-term care residents.

+ Work with a dynamic multidisciplinary team to assist in the planning of and direct provision of care; and performing all other related duties as assigned.

The Social Worker will:

meet with new admissions and their representatives promptly; provide orientation to facility, complete psycho social's, develop comprehensive care plans, provide support regarding changing health needs, and when appropriate, facilitate safe and appropriate discharges.

be capable of providing direct social work services and counseling to residents and families, individually or in a group setting as appropriate, according to current standards.

be capable of completing all documentation according to the Policies and Procedures of the Department of Social Services following the guidelines put forth by NYSDOH/CMC.

coordinate and attend weekly Interdisciplinary Comprehensive Care Plan meetings, discharge planning meetings, weekly staff meetings and mandatory in-services.

be capable of completing MDS+, PASARR SCREEN, PHQ 2-9's and other evaluations as required by regulation.

contribute to departmental performance improve projects.

understand and assist residents/representatives with understanding possible financial ramifications and have a knowledge base of Medicaid, SSI, Veterans or other benefits.

REQUIREMENTS INCLUDE:
