💼 Position Overview:
As an LPN Care Coordinator, you will play a pivotal role in managing and coordinating care for patients enrolled in Alopex’s services. You will work closely with patients, primary care providers, and other specialists to develop, implement, and monitor individualized care plans. Your goal is to ensure that patients receive the highest level of care through ongoing communication and coordination.

📝 Key Responsibilities:

• 🔎 Review patient medical records and assess Chronic Care Management (CCM) eligibility.
• 📞 Outreach to providers and patients to avoid duplication of services and assess participation in existing programs.
• 👩‍⚕️ Educate patients and providers about Alopex services and chronic condition management.
• 🗂 Develop and update individualized care plans for patients.
• 📆 Conduct bi-monthly care calls to address patient concerns and needs.
• 🌍 Survey social determinants of health and connect patients to community resources.
• 📋 Document patient information in detail, ensuring compliance with Medicare and Alopex's procedures.
• 🏅 Achieve monthly key performance indicators for high-quality care

✅ What You Bring to the Team:

• 🩺 Experience: Minimum 5 years of patient-centered care
• 💻 Tech Skills: Proficient in using software and EMR systems.
• 🔍 Clinical Expertise: Able to perform LPN-level assessments, problem-solving, and care plan development.
• ⏳ Time Management: Strong organizational skills and the ability to prioritize tasks effectively.
• 🗣 Communication: Strong verbal and written skills, with attention to detail.
• 🤝 Passion for Care: Patient-focused, compassionate, and dedicated to helping others.
• 🌐 Language Skills: English required; Spanish proficiency preferred.
• 🏅 Licensure: Current Compact LPN Licensure.

🏠 Remote Position Requirements & Schedule:

• 🌐 Reliable internet connection (all programs are internet-based).
• 🏡 Quiet, designated office space suitable for discussing sensitive patient information.
• 📜 Agreement to follow Alopex’s work-from-home policy.
• 🕘 Monday-Friday: 8:00 AM - 5:00 PM (or according to client practice hours).
